Position Title: Bilingual (Spanish/English) HIV Medical Case Manager
Reports to: Case Management Coordinator

Qualifications:
· Bachelor’s degree required, Master’s degree preferred and/or experience in social/human service delivery.
· Demonstrated expertise in case management and client advocacy.
· Ability to work with diverse populations (clients and staff) in critical and emergency situations.
· Sensitive to cultural, economic, gender, and sexual orientation differences.
· Bilingual Spanish/English
· Knowledge of local resources and service pool system. Understanding of medical and psychosocial issues related to HIV disease and related conditions.
· Flexibility in work sites, work hours, including occasional evenings and weekends.
Basic Function:
· Provide a full range of Case Management services to clients assigned to caseload of approximately 45-50 clients.
· Function as a part of the Direct Services team to ensure that high quality services are provided to clients.
Specific Responsibilities:
- Complete Intake in accordance with Uniform Access procedure
- Perform face to face needs assessments with clients, obtaining medical, demographic, and consent information.
- Assemble care plans together with client and other members of support team while ensuring confidentiality.
- Provide case management and advocacy by carrying a caseload of persons symptomatic with HIV disease.
- Available for home, hospital, clinic, and office visits as appropriate.
- Provide case management and back-up support to tenants of Casa Nueva Vida, our permanent subsidized housing program.
- Assist clients in negotiating social services arranging and ensuring linkage with other systems.
- Provide information and referral, including monthly follow-up and monitoring of related activities, and reassessment as needed.
- Provide information and education to members of clients’ support system as necessary when informed consent has been received.
- Consult with care providers in discharge planning and home care plans.
- Consult with Action Wellness Direct Service staff, volunteers, and therapist regarding specific clients’ plans as appropriate.
- Attend a minimum of 2 medical appointments.
- Provide intake coverage as assigned.
- Other duties as required.
Performance Standards:
The following performance standards are specific to this position. Meeting or exceeding these standards is an expectation of employment, as is compliance with all organizational and departmental policies, procedures and expectations.
- Maintain awareness and knowledge of resources, as well as benefits and entitlements necessary to appropriately refer clients for services and advocate for client needs.
- Document 450 units of service (15-minute increments) per month.
- Complete reports as determined by job function and department.
- Provide leadership and administrative and clinical back-up coverage for unit in absence of Case Management Coordinator.
- Assist in mentoring and training of staff and/or students in computerized client tracking system and Direct Service Procedures.
- Interface with outside collaborators and function as a liaison as dictated by specific job function to assist in coordination of services and procedures for clients to access services.
- Adherence to clinical and procedural policies in the Action Wellness Direct Services Manual
- Attend Action Wellness’ Direct Services, In-services, and General Staff meetings.
- Participate in weekly individual and monthly group supervisory meetings.
- Maintain accurate, timely records as required.
- Document the minimum contact with each client assigned to caseload per time frame outlined in the Direct Service Policy and Procedure Manual.
- Adhere to core values as outlined in Employee Handbook.
